The lot was a story the couple half-forgot they owned. Bought in the late 1980s for $18,000, an hour from home, it carried a promise to build a cabin someday. They never did. Their agent listed it on a Tuesday. A cash buyer closed the following Monday. A clean six-figure gain, a check to spread among the grandkids, and one loose end nobody mentioned at the closing table.

The sale happened in 2024. Late in 2025, Social Security notified them what it would add to their 2026 Medicare premiums. The land sold in a week. Medicare took nearly two years to send its bill.

The Two-Year Lookback Nobody Mentioned

Medicare premiums rise with income for a relatively small share of beneficiaries. The Income-Related Monthly Adjustment Amount (IRMAA) affects roughly 8% of people with Medicare Part B. The surcharge generally uses tax information from two years before the premium year. A property sale reported on a 2024 return therefore helps set the 2026 premium. By the time the notice arrives, the closing cannot be rescheduled and the income cannot be moved.

For IRMAA, modified adjusted gross income (MAGI) is adjusted gross income on Form 1040, Line 11, plus tax-exempt interest on Line 2a. Municipal bond interest counts even though it is exempt from federal income tax. So does the taxable gain from selling the lake lot. One-time income does not receive a special IRMAA exemption simply because it will not return the following year.

The Bill in Dollars

Assume the couple normally reports joint MAGI near $180,000 from Social Security, a pension, interest, and occasional Roth conversions. The lot sells for approximately $185,000. Against an $18,000 purchase price, the preliminary gain is $167,000, although commissions, legal expenses, and any additions to adjusted basis could reduce it. Using the full $167,000 for illustration, their MAGI reaches approximately $347,000.

Under the 2026 joint brackets, income above $342,000 and no greater than $410,000 produces a monthly Part B surcharge of $324.60 per person. Part D adds $60.40 per person. Together, the surcharges equal $385 per person each month, or approximately $9,240 for the couple over the year. If their income returns to normal in 2025, their premiums should fall again in 2027. The land sale creates a one-year Medicare aftershock, but $9,240 is still a costly loose end.

Why Form SSA-44 Does Not Fix It

Form SSA-44 can reduce IRMAA after certain life-changing events cause income to fall. Those events include retirement, divorce, the death of a spouse, and the loss of pension income. A voluntary property sale is not one of them. Social Security specifically treats one-time income and voluntary sales of income-producing property as nonqualifying events. Unless the couple experienced a separate qualifying event or Social Security used incorrect tax information, the surcharge generally stands.

Widows and widowers face another complication. After one spouse dies, the survivor may move from joint to single tax brackets, where IRMAA thresholds are roughly half as high. Similar income can therefore produce a much larger surcharge after filing status changes.

What to Settle Before the Closing

The time to find room around IRMAA is while the closing date and other income decisions can still move. Three questions deserve attention:

  • Which tax year should hold the sale? A December closing and a January closing produce different IRMAA years. If ordinary income will be lower next year, waiting several weeks could keep the gain in a lower tier.

  • What other income can stay out of that year? Postpone discretionary individual retirement account distributions or Roth conversions when possible. Required minimum distributions still must be taken on schedule.

  • Can the taxable gain be reduced or spread legitimately? Reconstruct the adjusted basis, include eligible selling expenses, look for capital losses elsewhere in the portfolio, and ask whether a seller-financed installment sale is practical. An installment sale can spread gain across multiple years, although interest and part of each payment still enter income.

The lot waited four decades to make them money. Medicare was simply two years behind it.
