Pharming Group N.V. (NASDAQ:PHAR) announced on September 11 that the U.S. Food and Drug Administration approved Joenja for children aged 4–11 with activated PI3K delta syndrome, or APDS, who weigh at least 27 kilograms. APDS disrupts immune function.

The new 40 mg and 50 mg tablet strengths expand dosing options for Joenja, or leniolisib. The pediatric regimen also includes 70 mg twice daily for children weighing at least 45 kilograms. Pharming Group N.V. (NASDAQ:PHAR) expects the new strengths in October through its existing distribution and patient-support network.

The expansion arrives as the revenue mix shifts. Second-quarter Joenja revenue increased 40% year over year to $17.9 million, while RUCONEST revenue declined 10% to $72.3 million. Total revenue fell 3% to $90.2 million. Joenja's growth is substantial, but the larger franchise still determines much of the overall result.

Bull Case

Pharming Group N.V. (NASDAQ:PHAR) can bring the pediatric doses to market through an established commercial platform. Distribution relationships and patient support are already in place, which could allow additional sales without duplicating the infrastructure required for an entirely new product.

The opportunity is to spread those capabilities across more eligible patients while maintaining support for existing users. Incremental revenue could improve commercial efficiency, although pediatric servicing costs will still matter.

There is an existing base to build on. Pharming Group N.V. (NASDAQ:PHAR) reported 132 U.S. patients on paid Joenja therapy as of June 30, five more than at the previous quarter-end. The pediatric approval creates another route to expand that population.

Further access expansion remains possible. Pharming Group N.V. (NASDAQ:PHAR) submitted a separate supplemental application on July 30 for children aged 4–11 weighing 13 kilograms to less than 27 kilograms. Approval of those lower doses could broaden eligibility beyond the latest label.

RUCONEST also showed sequential improvement, with second-quarter revenue rising 24% from the first quarter. Management expected stabilization and a return to growth in the second half of 2026. That recovery, if delivered, would reduce the burden on Joenja.

Bear Case

The identified pediatric population is not the immediately eligible population. Pharming Group N.V. (NASDAQ:PHAR) had identified 60 U.S. APDS patients aged 4–11 as of June 30, but the reviewed disclosures did not specify how many weighed at least 27 kilograms. Treating all 60 as potential near-term paid starts would overstate the disclosed opportunity.

Approval also precedes commercial conversion. October availability gives physicians and families a route to treatment, but prescriptions, coverage decisions and treatment initiation may take time. Revenue will depend on eligible patients starting paid therapy, continuing treatment and the net price realized.

The franchise imbalance remains significant. RUCONEST generated approximately four times Joenja's second-quarter revenue. Strong percentage growth from a smaller product can coexist with declining group sales, as the latest quarter demonstrated.

RUCONEST's year-over-year decline also had several drivers. Pharming Group N.V. (NASDAQ:PHAR) cited U.S. market dynamics, specialty-pharmacy inventory drawdowns, and its completed withdrawal from non-U.S. markets. Pediatric uptake must be assessed alongside those trends, rather than assumed to offset every source of weakness.

Conclusion

Pharming Group N.V. (NASDAQ:PHAR) has expanded access through a commercial platform that is already operating. The financial benefit now depends on the eligible patient count, paid pediatric starts, and incremental revenue. October availability is a practical milestone, but sustained Joenja growth and RUCONEST stabilization will together determine whether the portfolio returns to stronger growth.
