Biotech spent years as one of the market's least loved trades, and until this summer, the iShares Biotechnology ETF (NASDAQ:IBB) had trailed the broad market. Then it turned. Over the three months ending September 4, 2026, IBB returned 26%, while SPY returned 4% over the same window.

The gap forces a real portfolio question: if you missed the move, does IBB still earn a spot? IBB concentrates exposure to U.S. biotechnology, tilted toward established drugmakers rather than clinical-stage lottery tickets. Industry coverage has framed the rally as the market finally reckoning with a patent-cliff problem years in the making.

Patent Cliff Is Doing the Heavy Lifting

Big pharma faces a wall of branded-drug revenue rolling off patent through the end of the decade. Products generating billions in annual sales lose exclusivity, generics arrive, and revenue evaporates within quarters. Internal research cannot fill that hole on that timeline.

So they buy. Deal trackers have noted a marked uptick in pharmaceutical acquisitions as large buyers reach into mid- and small-cap biotech for late-stage assets, with generous premiums in announced deals. A buyer who needs a Phase 3 asset in 2027 to protect 2029 revenue is price-insensitive, and IBB's holdings are precisely the companies large pharma wants to own.

What You Actually Own in IBB

IBB holds 252 positions, but the weight is top-loaded. Vertex Pharmaceuticals (NASDAQ:VRTX) is near 8% of the fund, Amgen (NASDAQ:AMGN) near 8%, Gilead (NASDAQ:GILD) near 7%, and Regeneron (NASDAQ:REGN) near 5%. Those four names drive more return than the bottom half combined.

Because IBB is market-cap weighted, profitable large-caps dominate, and the speculative clinical-stage names contribute less than their count suggests. Compare against the SPDR S&P Biotech ETF (NYSEARCA:XBI), which is equal-weighted and therefore a more direct bet on small-cap clinical outcomes. XBI is up 34% year-to-date, ahead of IBB's 26%, which is what the higher-beta version of the same trade should do.

IBB charges a 0.44% expense ratio and held roughly $9.1 billion in net assets as of June 30, 2026. Treat IBB as a satellite sleeve of 3% to 7%.

Bear Case After a Run Like This

A fund that has already moved offers a worse entry than one that has not. IBB is up 51% over the last year, and XBI is up 76%. You are paying for the acquisition thesis rather than picking it up for free.

Clinical trials fail. A single Phase 3 miss at Vertex or Regeneron would take a real bite out of the fund given how concentrated the top weights are. Drug-pricing policy remains a live political risk, with every election cycle reintroducing the possibility that Medicare negotiation will expand to more categories.

Over five years, IBB returned 22% while SPY returned 70%. The recent quarter does not erase what was a punishing stretch for the sector.

Is IBB ETF a Buy?

Real economics underpin the rally. The patent cliff is a math problem for big pharma, and IBB's market-cap weighting harvests takeout premiums efficiently as they arrive.

If you have zero biotech exposure, a 3% to 5% starter position in IBB is defensible, scaled in rather than dumped in one order. If you already own biotech through a broad healthcare fund, adding IBB stacks you into large-cap biotech at exactly the moment the group has re-rated. And if you want higher torque and can stomach clinical volatility, XBI is the more direct instrument.

The thesis is intact, but the entry point has deteriorated enough that patience is warranted before adding exposure.
